Processes do not shut down, frequent Win Explorer shutdowns

Hi. This started gradually, and now virtually every time... I even reinstalled Windows, but it began again. When I cannot open Internet Explorer, Windows Media Player, and almost every other program now, I check task manager, and notice that there are several processes of these respective programs running, although I have closed them; I have to shut each one down manually. Also, Windows Explorer crashes often (and restarts). This has become such a nuisance that it is impossible to work. Is this malware? Here is the HijackThis log file:
