Right click on HijackThis and click Run as administrator
Click on do a system scan only
Place a checkmark next to these lines(if still present)

O2 - BHO: (no name) - {02478D38-C3F9-4efb-9B51-7695ECA05670} - (no file)
O2 - BHO: MS Video Control 1.0 - {E76AA4DA-7388-4D1E-B7E4-CC809F4E8F1B} - C:\Windows\msvidc32.dll

Then close all windows except HijackThis and click Fix Checked

Then please upload this file:

C:\Program Files\MegaForms Server\MegaVersion.exe

To either jotti or virustotal

Post back with the jotti/virustotal results, a new HijackThis log and a description of any remaining problems
