I'm not very icon wise either, I like clean desktop with the taskbar hidden.

Looks like Rainlender with a custom skin applied to it.

Its a cool little app. Can use it kind of like a day timer and set reminders for you on certain dates.
I use it as well.
I usually have quite a few apps running, that's why the 2 line taskbar.
