Trojan infection creating BITxxx.tmp files constantly

Thanks to some dodgey activex control, my computer is infected with some form of trojan horse. It keeps creating BITxxx.tmp files in my temp folder, you delete them, they keep coming back with a different "xxx" and the last one created cannot be deleted when on the internet because it is "in use by another person/program". Avast antivirus noticed it but obviously didn't stop it. I tried SDFix followed by Combofix which seemed to get rid of it, but the BITxxx.tmp files are still appearing at about 370KB each. Running ActivePorts I find that the BITxxx.tmp files are appearing everytime svchost.exe accesses or is accessed by svchost.exe by the IP address 81.29.248.50 (and no, this isn't my IP address). I'd be very grateful if anyone could help me remove this annoying trojan....hijackthis report is included.

Oh and I forgot to mention, the start menu is totally messed up too, looks kind of like a windows 95 start menu, and I imagine it's related....
