Malware Infection

I hope someone here can help me as I am having problems getting rid of what I think might be malware. Without my knowledge, my computer has downloaded an av.exe file which is currently sitting on my hard drive in my local settings folder. If I try and delete it, a warning comes up which tells me that it is a system file and if I continue, some of my programs may not work properly. I went ahead and deleted it anyway then, when I tried to run Malwarebytes, a box popped up saying that Windows could not open this file, gave me a list of programs and asked me to select the appropriate one for this program. When I undeleted the av.exe file, I was able then to run Malwarebytes, after renaming it as suggested in some other websites I visited. (It would not run as mbam.exe because the av.exe was stopping it, so I renamed it myapp.com.)

I have tried several ways to get rid of this av.exe, which is manifesting itself as Windows Internet Security 2010, but so far nothing has worked. This is the information I got when I installed and ran HijackThis:
